HNF also looks after patient’s social well-being and other practical daily needs, bringing care and support into the homes of those most in need. This is done through a multi disciplinary, case management approach and delivered by its highly qualified teams comprising of medical, nursing, allied health and social welfare professionals. On average, HNF serves close to 5,000 patients, making almost 38,000 home visits annually.
We aim to provide high quality, comprehensive and affordable nurse-led home healthcare services. We will do this through care-coordination, caregiver training and education. We will deliver our service to the best of our ability, with compassion and respect, and in a caring and professional manner.

Intended use of funds through the YFC 2018 programme
As Home Nursing Foundation also looks after the patient’s social well-being and other practical daily needs, the funds raised will help to defray the cost of the nursing/medic fees for patients who are in the financial assistance. Funds raised will also help to provide additional support to lower-income patients, so they are able to lead fulfilled and dignified lives during their recuperating processes.
Engaging and involving the YFC participants
HNF will engage the participants by educating them about our services, and how it is run on a daily basis as well as our patients’ stories over at our office. To allow the participants to have a better understanding of the patients whom HNF serves, patients’ homes visit will be arranged. Through the home visit, participants are able to interact with the beneficiaries and hear more about their recovering journey and the different challenges they faced in life. HNF will also guide and work closely with the participants t come up with interesting fundraising activities to engage various stakeholders.
