Home Nursing Foundation
Founded in 1976, HNF is the largest and most established home healthcare service provider in Singapore. It is also the only provider that operates islandwide in Singapore. HNF delivers comprehensive care programmes that are dedicated to our patients, regardless of their income level. Our services include Home Nursing, Home Medical, Home Therapy and Social Support. From nursing to medical care, to looking after patients’ social well-being and other practical daily needs, we bring care and support into the homes of those in need.
These services are carried out by a team of professional Nurses and affiliated General Practitioners (GPs), Therapists and Medical Social Workers who make regular home visits. For patients who require social services such as food delivery and personal hygiene care, we help them get in touch with the appropriate providers.
Home is the best place to recover from or cope with an ailment. At HNF, we embrace our patients with all-round support they can depend on. We start by understanding our patients’ needs and by adopting a multidisciplinary case management approach. With our nursing and medical personnel, and network of partners, our holistic patient-centric home healthcare plans are individually tailored to meet the needs of each patient.

Intended use of funds through the YFC 2019 programme
The funds raised will go towards subsidizing our patients’ out-of-pocket fees and supporting our operational needs.
HNF runs on a combination of government subsidy, donations, and fees collected from patients. HNF receives government funds as subsidies for Home visits to patients, according to each patient’s means-tested tier. The maximum government subsidy is 80% of the government’s reckoned cost – the ‘funding rate’.
HNF’s charges are set internally, as we balance keeping charges affordable with keeping HNF afloat. Where a patient cannot afford to pay the balance of HNF’s charges after government subsidy, HNF may assist the patient to apply for MediFund subsidy. If the patient is not eligible for MediFund subsidy, HNF draws on charity dollars. (This is a simplified picture of funding and subsidies, and thus does not contain elaboration on fine details.)
Engaging and involving the YFC participants
We would invite them to our office for an introductory session on Home Nursing Foundation and our beneficiaries. We will also arrange for patient home visits led by our nurses and therapists, so they will have a better understanding of our patients, their needs, and the home care services provided to them.
